All accidents will be reported to the nurse in writing on the Student Injury Report form. This form must be completed on the day of the accident. The report must list the name of the student, how the accident occurred, names of witnesses, and the signature of the supervising adult. It is the responsibility of the teacher to be sure parent contact is made. The actual contact can be made through the office.

Immunization, Eye, Ear, and Dental Records

According to state law, all students entering Georgia Schools, regardless of grade level, need to show evidence of having had the proper immunizations as well as eye, ear, and dental examinations. The school must have complete records or request that the child be withdrawn from school. The examinations must have been completed within the previous year. These certificates must be kept on file at the local school level. Refer any questions to the school nurse.

Medicines

All medications for students should be placed in the office by an adult and dispensed by the school nurse. Written permission must be on file for medication to be administered. All medication must be in the orginial container.

Too sick for school?

Early in the morning it is often difficult to make a decision about whether or not your child is sick enough to stay home from school.  With minor symptoms you often cannot tell whether he is going to get better or worse during the course of the day.

The main reasons for keeping your child home are:

  • If he/she is too sick to be comfortable at school.
  • If he/she might spread a contagious disease to other children.

Remember to always make sure that the school knows how to reach you during the day, and that there is a back-up plan and phone number on file if we cannot reach you.

Parents will be called & students sent home with: lice/nits, temp. of 100.4 or greater, vomiting, diarrhea, wide spread rash, persistent pain, frequent cough, and/or other as determined by Nurse.

Lice Policy

Students who have been sent home should receive prompt treatment for lice/nits and return to school the following day.  Any absences due to head lice/nits are unexcused.  When returning to school they must be brought in by an adult to the front office, they cannot ride the bus until cleared by the school nurse.  If child is found lice/nit free they will be returned to their regular school day and be allowed to participate in all school activities including riding the bus. Special Health Care Need and Allergies

If your child will be returning to school with any special health care needs, i.e. cast, crutches, special diet, newly diagnosed illness, blood sugar monitoring, nebulizer treatments, etc. please contact the school as soon as possible to discuss accommodations.
