Medication Administration Guidelines
Please review the following guidelines for medication administration during school hours. The information below can also be found in your child's agenda. With your cooperation on this matter we can together keep your children's health and well being a top priority.

All medication (prescription and non-prescription) must be taken (by an adult) directly to the office or clinic for safe storage. |

All medication must be accompanied by an administration of medication request form signed by a parent/guardian. School officials will not assist in the administering of any medication without signed permission. There will be no exceptions. Verbal consent cannot be accepted. |

All medication must be in the original container. Prescription medicines must be labeled with the student's name and will be given according to labeled instructions on the bottle. If medicine is needed during school hours, during a limited time-period, bring enough medicine for daily doses. No medications can be returned to a student to carry home. Unused medication will be stored in the clinic until an adult can pick it up. |

Any medication found in the possession of a student will be taken to the clinic for storage until the parent/guardian can be contacted. It will not be returned to the student, and the student will face disciplinary action for violation of the school's policy on medication administration. |

All medications stored at school must be picked up the last day of school each year. The school nurse will dispose of any medication left in the clinic a week after school ends. |
