All accidents will be reported to the nurse in writing on the Student Injury Report form. This form must be completed on the day of the accident. The report must list the name of the student, how the accident occurred, names of witnesses, and the signature of the supervising adult. It is the responsibility of the teacher to be sure parent contact is made. The actual contact can be made through the office.

Medicines

All medication for students should be placed in the office by an adult and dispensed by the school nurse. Written permission must be on file for medication to be administered. All medication must be in the original container.

Immunization/Eye,Ear and Dental Records

According to state law, all students entering Georgia Schools, regardless of grade level, need to show evidence of having had the proper immunizations as well as eye, ear and dental examinations. The school must have complete records or request that the child be withdrawn from school. The examinations must have been completed within the previous year. These certificates must be kept on file at the local school level. Refer any questions to the school nurse.
