Registration

New student registration: June 9, 2009 - July 31, 2009.

We will register students each Tuesday and Thursday from 9:00 am until 1:30 pm.
(Office will close 11:30-12:00 for lunch each day.)

IMPORTANT IMMUNIZATION INFORMATION: ALL CHILDREN ENTERING 6TH GRADE (ON OR AFTER July 1, 2008) ARE REQUIRED TO MEET THE FOLLOWING NEW REQUIREMENTS:

Each student should have:

• two (2) doses of Measles vaccine
• two (2) doses of Mumps vaccine
• one (1) dose of Rubella vaccine or laboratory proof of immunity against Measles, Mumps or Rubella.
• two (2) doses of Varicella (chicken pox) vaccine or documentation of disease or laboratory proof of immunity. (At
  the time your child entered school only one (1) dose of this vaccine was required.)

Contact your health care provider to receive any needed vaccinations and to obtain the Georgia Certificate of Immunization (Form 3231).

Parent's Checklist

The person enrolling the student must be the legal parent or guardian; this person will have to sign all registration forms.

Please be aware of Buford City Schools' registration policy.  If a student registers on Monday before 12:00 am, they can begin school on Tuesday morning. If they register after 12:00 am, they will have to wait until Wednesday morning to begin school.

Please note that Buford Middle School is a "standardized dress code" school. Please see our Dress Code website for more details.

You will need the following information before you can register your student:

• Any records from the student's previous school:  Withdrawal form, current grades (Progress Report or Report Card), Discipline and Attendance record, Immunization Record, Hearing, Dental & Vision Screening Record.

• Copy of Certified Birth Certificate - No cards accepted.

• Copy of Social Security Card.

• Updated copy of Immunizations on GA Form 3231 - You can obtain this information from your child's doctor, Buford Health Department (770) 614-2400, or the previous school.

• Copy of Court Order Custody (if applicable).

• Registration Form - Complete with legal parent or guardian signature.

• Records Release Form - Regular and Special Education (if applicable) with legal parent or guardian signature.

• Residency Affidavit - Proof of Residency - Residency is within the city limits of Buford.  If unsure, please call Buford City Hall at (770) 945-6761.

• Any additional addendums that may apply to the current school year.

• $5.00 Agenda fee.

• $5.00 Locker fee.
